Terry L. Cline, Ph.D. began his tenure as Oklahoma s Commissioner of Health effective June 30, 2009. Dr. Cline was appointed to this position by the Oklahoma State Board of Health and he is the 20th State Health Commissioner to serve since the position was first created in 1907. In February 2011, Gov. Mary Fallin appointed him to serve concurrently as Oklahoma s Cabinet Secretary of Health and Human Services.

Dr. Cline previously completed a post as Health Attach at the U.S. Embassy in Baghdad, Iraq, where he advised the U.S. Ambassador, the Iraqi Minister of Health, and the U.S. Department of Health and Human Services on health-related challenges in Iraq. Dr. Cline served in this capacity under the Administrations of both President George W. Bush and President Barack Obama.

Prior to that position, Dr. Cline served as Administrator for the federal Substance Abuse and Mental Health Services Administration (SAMHSA) from 2006-2008, a position for which he was nominated by President George W. Bush and confirmed by the U.S. Senate. As SAMHSA Administrator, Dr. Cline directed the $3.3 billion agency that is responsible for improving the accountability, capacity and effectiveness of the nation s substance abuse prevention, addictions treatment, and mental health service delivery systems.

Dr. Cline has extensive experience in overseeing health and human services at the state level as well. In 2004, he was appointed by Gov. Brad Henry as Oklahoma s Secretary of Health. At the same time, he served as Oklahoma s Commissioner of the Department of Mental Health and Substance Abuse Services, a position he held since January 2001.

Dr. Cline has also served as a local provider through an earlier post as the Clinical Director of the Cambridge Youth Guidance Center in Cambridge, Mass. and as a Staff Psychologist at McLean Hospital in Belmont, Mass. In addition, his professional history includes a six-year appointment as a Clinical Instructor in the Department of Psychiatry at Harvard Medical School and Chairman of the governing board for a Harvard teaching hospital in Cambridge, Mass.

A native of Ardmore, OK, Dr. Cline attended the University of Oklahoma where he earned a bachelor s degree in psychology in 1980. He then received both a master s degree and a doctorate in clinical psychology from Oklahoma State University. Dr. Cline has involved himself in community service, including membership on a number of local, state and national committees and boards with a focus on improving the overall health of the community and the nation.

Adrenalin (epinephrine ) is a chemical that narrows blood vessels and opens airways in the lungs. These effects can reverse severe low blood pressure. wheezing. severe skin itching, hives, and other symptoms of an allergic reaction. An injection of adrenalin is used to treat severe allergic reactions (anaphylaxis ) to insect stings or bites, foods, drugs, and other allergens. Side effects of Adrenalin include:

Adrenalin is available in many different injectable forms including auto-injector systems that can be carried by those who are prone to serious allergic reactions. The dose for adults and children 30 kg (66 lbs) or more is 0.3 to 0.5 mg (0.3 mL to 0.5 mL) of undiluted Adrenalin administered intramuscularly or subcutaneously in the anterolateral aspect of the thigh, up to a maximum of 0.5 mg (0.5 mL) per injection, repeated every 5 to 10 minutes as necessary. Adrenalin may interact with digoxin, diuretics (water pills), levothyroxine. chlorpheniramine or diphenhydramine (antihistamines commonly contained in cold, allergy. or over-the-counter sleep medications), antidepressants. beta-blockers, ergot medicines, heart rhythm medications, or MAO inhibitors. Tell your doctor all medications and supplements you use. There are no adequate and well-controlled studies in pregnant women. Tell your doctor if you are pregnant before using Adrenalin. Consult your doctor before breastfeeding.

Adverse Reactions Associated With Intramuscular/Subcutaneous Use (for Anaphylaxis)

Common adverse reactions to systemically administered epinephrine include anxiety, apprehensiveness, restlessness, tremor. weakness, dizziness, sweating, palpitations. pallor, nausea and vomiting, headache, and respiratory difficulties. These symptoms occur in some persons receiving therapeutic doses of epinephrine, but are more likely to occur in patients with heart disease. hypertension. or hyperthyroidism [see WARNINGS AND PRECAUTIONS ].

Due to the lack of randomized, controlled clinical trials of epinephrine for the treatment of anaphylaxis. the true incidence of adverse reactions associated with the systemic use of epinephrine is difficult to determine. Adverse reactions reported in observational trials, case reports, and studies are listed below by body system:

Cardiovascular: angina. arrhythmias, hypertension, pallor, palpitations, tachyarrhythmia, tachycardia. vasoconstriction. and ventricular ectopy.

Arrhythmias, including fatal ventricular fibrillation. have occurred, particularly in patients with underlying organic heart disease or patients receiving drugs that sensitize the heart to arrhythmias [see WARNINGS AND PRECAUTIONS ].

Rapid rises in blood pressure associated with epinephrine use have produced cerebral hemorrhage, particularly in elderly patients with cardiovascular disease [see WARNINGS AND PRECAUTIONS ].

Respiratory: respiratory difficulties.

Neurological: dizziness, disorientation, excitability, headache, impaired memory, lightheadedness. nervousness, panic, psychomotor agitation, sleepiness, tingling, tremor, and weakness.

Psychiatric: anxiety, apprehensiveness, restlessness.

Gastrointestinal: nausea, vomiting.


Patients with Parkinson’s disease may experience psychomotor agitation or a temporary worsening of symptoms [see WARNINGS AND PRECAUTIONS ].

Diabetic patients may experience transient increases in blood sugar [see WARNINGS AND PRECAUTIONS ].

Accidental injection into the digits, hands or feet may result in loss of blood flow to the affected area [see WARNINGS AND PRECAUTIONS ]. Adverse events experienced as a result of an injection into these areas include increased heart rate, local reactions including injection site pallor, coldness, hypoesthesia, and tissue loss, or injury at the injection site resulting in bruising, bleeding, discoloration, erythema. and skeletal injury.

Injection into the buttock has resulted in cases of gas gangrene [see WARNINGS AND PRECAUTIONS ].

Rare cases of serious skin and soft tissue infections, including necrotizing fasciitis and myonecrosis caused by Clostridia (gas gangrene), have been reported following epinephrine injection in the thigh [see WARNINGS AND PRECAUTIONS ].

Adverse Reactions Associated With Intraocular Use (for Mydriasis)

Epinephrine containing sodium bisulfite has been associated with corneal endothelial damage when used in the eye at undiluted concentrations (1 mg/mL).

USDA Rural Development Mortgage

With the USDA Rural Development option, you can borrow 100% of the appraised value. This means you don’t need to come up with the down payment. In fact, you can actually borrow 2% more than the appraised value, and use the extra to make some light repairs or home improvements. The main requirement for a USDA Rural Development mortgage is that the property must fall within certain geographical areas, outside the city limits of major metropolitan centers.

Now, this doesn’t mean you have to live “way out in the country.” While that is an option if it appeals to you, many areas that fall under the Rural Development umbrella are actually more suburban than you’d expect. As an example, rural development in northern Indiana covers most areas except Gary, South Bend and Fort Wayne. Other villages, towns and areas are eligible.

Under the Guaranteed Loan program, USDA Rural Development guarantees loans made by private sector lenders like AmeriFirst Home Mortgage. A loan guaranteed through RD means that, should the individual borrower default on the loan, RD will pay the private financier for the loan. You work with AmeriFirst and make your payments to us.

The only disparaging issue is the Government operates on a fiscal year budget that ends on September 30th of every year. It can take a month or much more after September 30th for the new budget to be allocated. It is also possible to deplete the budget prior to Sept 30th, as it has in the past. Your loan could be held in limbo, with out closing, if it falls into this time frame.

Lenders usually have the option to close the loan without budget if they are issued a Loan Note Guarantee – Subject to Fund Allocation . Most lenders will not close with a subject to LNG. If this happens it becomes a waiting game.

    The USDA Rural Development Home Loan is a flexible zero down payment government guaranteed program that is growing in popularity. It is designed to promote homeownership to residents in rural communities with low to moderate incomes and who have limited savings for a down payment. A common misconception about the USDA loan program is that it is only for farmers, but you will find that just outside most metropolitan areas there are many suburban areas that qualify for this program.

    The Section 502 Guaranteed Loan is the most common type of USDA rural housing loan. Amazingly, this loan will actually lend up to 103.5% of the home’s appraised value and even allow the buyer to include closing costs in the actual loan (appraisal permitting). All USDA Guaranteed Loans carry a 30 year term with a low fixed rate.

    USDA Home Loan requirements are not entirely credit score driven, although RANLife Home Loans require a 640 mid-score or better, USDA Home Loan guidelines will disregard some credit derogatoriness with an acceptable explanation.

    The home must be owner occupied (no investment property) and all single family, condos and planned unit developments. NO MANUFACTURED OR MOBILE HOMES.

    You must be discharged from a Chapter 7 bankruptcy for at least three years. If you are in a Chapter 13 bankruptcy and have made all court approved payments on time and as agreed for at least one year, you are eligible to make an USDA loan application.

    The home must be owner occupied (no investment property) and all single family, condos and planned unit developments (PUD). NO MANUFACTURED OR MOBILE HOMES.

    One of the biggest advantages of a USDA RD Home Loan is a the very low mortgage insurance (MI) requirement. This alone will potentially save you $50-$250/mo depending on your loan size.

    However, USDA has been recently changing the MIP requirements. Call a RANLife USDA specialist today to see how the changes might affect you.

    USDA RD Home Loans have no down payment requirement. FHA requires 3.5% down and conventional loans require 3% down.

    To qualify for this loan program, there are two notable requirements that differentiate this program from an FHA or VA loan program.

    1. Location: The home must be located in a designated rural area.

    2. Income Limits: Must meet USDA adjusted annual household income limits, a maximum 115% of the median income for your area. Meaning your total combined household income cannot be more than this amount. If your income is slightly over these amounts, there are little known deductions that can be used to reduce your qualifying gross household income and help you qualify. Such as:

    • Disabled or handicapped individuals who are not the applicant or co-applicant.
    • Documentable childcare expenses for children 12 years of age or older.
    • Documentable medical expenses for family members 62 years of age or older.
    • Attendant care expenses.
    • Deduction for each child under the age of 18 and/or full time student over 18.

    Qualifying Income: It is important to note that USDA uses two types of income for qualifying.

    • Household income is the combined adjusted gross income of all people living in the home, regardless if they are applying for or will be on the mortgage. This amount cannot be higher than the county limits.
    • Repayment income is income from the actual loan applicants and determines the DTI (debt-to-income) ratio. Deduction for each child under the age of 18 and/or full time student over 18.
